To reduce your wait time and help with efficiency, please schedule an appointment as far in advance as possible. Appointments are confirmed by the office staff a day before the scheduled appointment. If you are unable to keep your appointment, please call. If appointments are not cancelled within 24 hours, a no show fee of $50 will be added to your account. Late arrival may necessitate rescheduling your appointment.

Patient forms are available to help facilitate and expedite your visit at our office. Simply print, complete, and sign the appropriate forms and give it to our staff upon arrival.

To submit a perscription request, please provide us with your name, dose, quantity, when you take it, and your pharmacy name and number. Please note that narcotic medications cannot be refilled over the phone.

Although requests are handled in a timely manner, we require a 48 hour notice. 

Copies of medical records must be requested in writing. Patients may request a copy of their medical records one time, free of charge. Legally, medical offices have up to 30 days to complete requests for records. However, we put forth every effort to respond to requests within days of receipt. Please allow up to 72 hours. 

We understand that at times, various forms or letters may be required to assist you with your healthcare needs. Due to the time-consuming process, fees for this service may apply. Cost is discussed in advance and prepayment may be required. 

Financial Policy

Per insurance company policy, all office co-pays are to be paid at the time of service. We are unable to assist you if co-pay is not paid at the time of service. 

If a referral from your primary care physician is required by your insurance, it is your responsibility to procure the referral. We are unable to assist you if a referral is not present during your visit.

Patients are responsible for any co-insurances, deductibles, and any other 
non-covered billable services. Please notify our office of any changes in insurance coverage or demographics. 

Travel Medicine
We do not accept insurance for travel or vaccine related services. Full payment for travel services is due at the time of visit. However, we are happy to provide you with a receipt and itemized statement that you may submit to your insurance for reimbursement, if applicable. 


Balances are due within 30 days of when the bill is issued. Bills will be issued after the insurance carrier pays its portion of the bill. Acceptable payment methods include checks, cash, and credit cards. In addition to paying by mail, you may call the billing office during business hours with your credit card information at 914-631-1415.
